The Licensed Practical Nurse (LPN) is responsible for overseeing and coordinating the healthcare needs of individuals supported by the organization. This position provides direct nursing care, administers medications and treatments, monitors health status, and ensures accurate clinical documentation. The LPN collaborates with physicians, registered nurses, therapists, families, and Direct Support Professionals (DSPs) to develop and implement individualized care plans that promote optimal health outcomes. Responsibilities include staff training, health education, medical appointment coordination, hospital follow-up, emergency response, and advocacy to ensure continuity of care. Prolonged periods of sitting at a desk and working on a computer. While performing the duties of this job, the employee is regularly required to talk and hear.
