Overview

The Intake Specialist plays a critical role in supporting individuals with intellectual and developmental disabilities through the full-cycle intake and enrollment process within the New Jersey DDD and NY OPWDD system. This position ensures that all documentation, coordination, and communication are handled efficiently and in compliance with state regulations and organizational standards.

Responsibilities

Intake & Enrollment Coordination

  • Manage intake for prospective individuals entering residential, day habilitation, or vocational programs.
  • Coordinate assessments, tours, and admission meetings with families, guardians, and care teams.
  • Gather and verify eligibility documentation including NJCAT assessments, medical records, and SDRs.
  • Initiate enrollment communications by sending NJ ISP and SDRs to the Data Integrity Specialist and relevant program teams.
  • Maintain the referral data spreadhseet, complete necessary documentation required by DDD/OPWDD (PRF, Screening Forms, CRO forms etc.)
  • Attend and participate in required meetings, workshops and training.

Documentation & Compliance

  • Ensure all required documents are uploaded and linked to service authorizations.
  • Maintain accurate records in compliance with HIPAA, licensing, and NJ DDD/NY OPWDD standards.
  • Collaborate with Residence Managers to complete Individual Demographic Forms (IDFs) and ensure guardian/disability information is entered correctly.

Stakeholder Communication

  • Liaise with referral sources including social workers, support coordiantors, care managers, state representatives and schools.
  • Educate families and guardians on services, rights, and expectations.
  • Respond to follow-ups and escalate missing documentation per NJ DDD policy.

Transition Planning

  • Work with program and support staff to ensure individualized plans and accommodations are in place prior to admission.
  • Support seamless transitions into programs by coordinating with internal teams and external providers.
  • Perform other duties as assigned or requested by the leadership team.

Qualifications

  • Associate’s or Bachelor’s degree in Social Work, Healthcare, Special Education, or related field
  • 2+ years of experience in admissions, case management, or disability services
  • Familiarity with NJ DDD systems, policies, and documentation workflows preferred

Pay Range

USD $2,000.00 - USD $2,115.39 /Bi-Weekly
